Map of Medicine Medication Policy
Map of Medicine does not routinely specify the doses of medications within its care maps. The responsibility for the prescribing of medications lies with the clinician.
On occasion, however, Map of Medicine will include a dose if one or more of the following criteria are met:
- Failure to include a dose may create ambiguity and significantly increase the risk of an adverse drug event or a prescribing error
- Evidence suggests that the type and dosage formulation of a particular medication is significantly prone to errors
- Stating a specific medication dose from the most up-to-date clinical practice guidelines may help achieve clarity
In every instance, Map of Medicine always recommends any doses are checked with the local formulary, and verified against the manufacturer's guidance as appropriate.
Generic and brand names are used only if they are specified by the evidence base, otherwise medications are referred to by class name.
